We will need some basic information to get started:

  • A prescription or written order from your physician for the products and services that you require

  • Basic demographic information including your address and phone number

  • Your insurance information including the name of the insurance provider, the Group and plan numbers that are listed on your insurance card.

  • Please call us if you need assistance with any of this information.​
