SENDING AN ORDER

 

If you have a patient that you would like us to serve, please send their information to us using our fax number:  (713) 218-0774. 

We care for all patients that require any form of Respiratory Therapy or Sleep Therapy. 

In all cases, we will need a Detailed Written Order to get started.  A Detailed Written Order must include the following items:
 

• Beneficiary Name

• Physician Signature, printed name and NPI

• Date

• Detailed description of the device being ordered including settings and frequency of use

• List of accessories or supplies dispensed with this equipment

• Refill / supply replacement instructions

• Length of need

• Qualifying diagnosis

 

In addition, we will need clinical documentation to support medical necessity.  For a Sleep Therapy device such as a CPAP or BiPAP, we will need sleep study results in addition to the detailed written order.  For a ventilator, we will need clinical notes to support the qualifying diagnosis.
